Immunization Requirements:
You can now browse by grade level (Child Care, K-12, 7th grade, and College) through the link below. Every grade level includes information on immunization requirements, tools for schools (including login for annual reporting), and materials for parents and students. We also added a laws section where you can find regulations, information about exemptions, as well as records, forms, and materials. A new email archive of school-related communications from CDPH can be found on the web page below.
Pertussis Information:After the 2011-2012 school year, the requirement will be that all students entering 7th grade provide proof of at least one Tdap booster given on or after their 10th birthday. Please discuss this with your child’s health care provider and provide a copy of their updated immunization record to the school prior to the start of next school year.
